Promo Mechanics

To join the monthly and grand raffle draws, consumers must send in as entry, one (1) empty box of Unique Toothpaste 50m L, or 100 mL, or 150 mL. The whole box should be submitted as entry bearing the brand name and grammage.
2.
Raffle entries must have the complete name, mailing address, telephone number (if any) and the signature of the sender written at the back of the Unique box.
3.
Entries can be dropped in the designated drop boxes located at major supermarkets and grocery stores nationwide or mailed to ACS Manufacturing Corporation. Entries to be mailed should be placed in envolopes addressed to:

Entries will be accepted beginning April 1, 2007 and shall be accumulated until October 24, 2007. Only those submitted within this period may quality.

The monthly draws will be held on the last Friday of every month beginning June 29, 2007. Each monthly draw will be drawn in the presence of a DOH-BFAD Representative. The grand draw will be held on October 21, 2007, again in the presence of a DOH-BFAD Representative. The monthly raffle draws and the grand raffle draw will be held at ACS Carmelray (Carmelray Industrial Park I, Canlubang, Calamba City, Philippines).
